In order to avoid this kind of difficulty, it is necessary to publish in the gazettes that a person with different names in different documents is one. The entire step by step process for this is as follows.
Regarding change of name/surname, the adult applicant has to sign the recent passport size photo along with the notice form. The affidavit for the purpose of publication in the gazette must be submitted in original with the signature coin of the First Class Magistrate / Executive Magistrate or Notary on non-judicial stamp paper for publication in the Gazette.
